For Referrals and Determination

Once a referral has been made to ACS, a program coordinator will conduct an intake interview to identify an individual’s strengths, needs, and program eligibility. If it is identified that services with ACS are not available, a referral will be made to an outside community agency. Click here to make a referral today.

For MH Admission

Individual must have a primary diagnosis of Serious Mental Illness which can also include:
  • Discharging from state hospitals
  • History of sex offenses and/or identified on the current sex offender registry. (These will be reviewed on a case-by-case basis)
  • Significant legal history and/or involvement which may include individuals found Not Guilty by reason of Insanity. (NGRI).

ID/DD Admission

To be admitted to the residential program, the individual:
  • Must have a documented ID/DD diagnosis.
  • Must be in need of support and/or training to live independently.
  • Must be over the age of 18
  • Must have specific identified characteristics:
    • ADHD
    • Oppositional Defiant Disorder
    • Learning Disabilities
    • Speech/Language Impairment
    • Primary Diagnosis of Mental Illness
